1. What is the DDR ID card?

The DDR ID card is an identification card that is generated through the Developmental Disability Registry (DDR) for the safety and protection of persons with disabilities and seniors with dementia.  The ID cards helps cardholders to minimise them from being victims of wrongful accusation and provide a source of identification when they are lost.  Parents and caregivers can be at greater ease when persons with disabilities and seniors with dementia are unaccompanied outside of their homes.

DDR refers to the Developmental Disability Registry (DDR), an electronic system used by Voluntary Welfare Organisations (VWOs) to effectively manage client’s information and monitor client’s development over time. DDR can further aid VWOs in their collaborative efforts in the provision of coordinated services to clients.

2. What are the uses of the DDR ID card?

In situations where persons with disabilities and seniors with dementia are unable to inform the authorities of the contact details of his/her parent/caregiver,the information printed on the card will be helpful.

The contact of the social service agency on the card will be useful for the police to enlist their help as required. Medical personnel will know whom to contact in an emergency and information on the card will enable them to provide appropriate medical intervention when treating persons with disabilities and seniors with dementia.

4. What information is captured on the DDR ID card?

The following information pertaining to the cardholder is reflected:

  • Name and photo
  • Date of birth
  • Gender
  • Contact of the social service agency
  • DDR ID card serial number
 
Parents and caregivers also have the option to include the following information on the DDR ID card:
  • Residential address
  • Residential and/or emergency contact number of parents/guardians
  • Language spoken by the cardholder
  • Special needs of the cardholder

5. Who can apply for the DDR ID card?

Persons with disabilities and seniors with dementia who are Singapore citizens or Singapore permanent residents can apply for the DDR ID card.

6. Where can I apply for the DDR ID card?

Service users may apply for the card at their participating agencies registered on DDR. 

For those who are not receiving services from the listed VWOs, please contact the following for the issuance of a DDR ID card. Applications must be accompanied with an assessment report certified by a medical doctor or therapist who has confirmed the applicant's condition.

7. Does the DDR ID Card entail a fee for subscription and for replacement of card?

NCSS will issue the first DDR ID card free-of-charge to clients through participating DDR-registered agencies.  Subsequent replacement cards issued may be chargeable at $2 each.

8. Is the DDR ID card transferable?

The DDR ID Card is not transferable and may only be used by persons to whom the card is issued to.
