| The DDR ID Card was introduced with the aim of helping persons with disabilities and seniors with dementia to integrate into society and live independently to the best of their ability. With the card, cardholders can also enjoy discounted rates for services and access to facilities offered by participating companies. |

- Application should be made through the social service agency that had issued the DDR ID card. A photocopy of the applicant’s DDR ID card may be requested at NCSS’s discretion.
